Dr. Vinod K. Shah came to Balancelogic with the idea to create a booklet that his fellow doctors could hand out to patients where it could educate them on what Chronic Care Management is, the benefits, and how MedStar Shah Medical Group can help them.
Balancelogic stepped up to this challenge and designed an 11″ x 11″ Booklet that included monthly note pages so the patient can write down important information from their visit that day, as well as a pocket so doctors can easily insert flyers or other important documents in the booklet without worrying about them falling out while the patient carries it. Balancelogic also designed the slides for a presentation that Dr. Vinod K. Shah was conducting to present to other local doctors to show them how they can also provide Chronic Care Management to their patients.
